INDEPENDENT CARE WAIVER PROGRAM
The Independent Care Waiver Program (ICWP) is for Medicaid eligible recipients with physical disabilities to receive personal home health care services; this is an alternative to a healthcare facility (Nursing Home or Hospital). This waiver is also available to persons with traumatic brain injuries (TBIs). The following are requirements for the program:

Individuals between the ages of 21 and 64

Approved for Medicaid

Individuals with severe physical disability
SERVICES PROVIDED
Our Case Management services lead with advocating for individuals with disabilities by ensuring their needs and rights are understood and respected, connecting them with appropriate resources and services, collaborating with healthcare and support professionals, facilitating access to barrier free services. This service may be used to locate, coordinate, and monitor necessary and appropriate services, including:
