Care Bridge case managers act as advocates for individuals with disabilities, ensuring their rights, need, and preferences are recognized and respected within the social service system and their communities.
We educate individuals about their rights, available resources and support services, ensuring they have the knowledge to make informed decisions about their care and well-being. We share information, guidance, and training to individuals and families on various aspects of disability management, self-advocacy, and accessing community resources.
We serve by offering emotional support, empathy, and encouragement to individuals with disabilities, helping them navigate their challenges and resources. We consistently monitor the effectiveness of the providers and services, and make necessary adjustments to the care plan to ensure the best possible outcomes.
